From what little is in your log, it appears as if the disk is having problems ... that could be from a faulty disk or a slot issue.
Have you run Online Diagnostics on it - or any of the other drives?
Your PERC firmware is out of date, and some of your drives as well - I'm not suggesting you do that until all has been repaired, but it might be crucial that you do that.
Have you ever run a Consistency Check?
You might also consider rebuilding in another slot, if you have one available.
