I spent several days experiencing similar frustration to what you are going through, but i systematically worked through all the possible combinations of the sequence of steps involved. That first 5450 must have been in and out of my PowerEdge fifty times as one by one I identified incorrect install methods. Several of my attempts caused unacceptably long boot times, so I crossed them off and kept going. Without knowing exactly what you've done, you can't be sure what's caused it or ensure that you don't repeat it.
